How Do You Act When The World Is Watching?

In June 2014 I had the privilege of doing a 2 week medical rotation at the US Olympic Training Center in Colorado Springs.  Being a chiropractic physician specializing in sports medicine, this was a big deal for me.  It’s something I set goals for, worked hard to develop my skill-set, thought about and dreamed about for a very long time.  While I was there treating America’s current and future Olympians and treating athletes alongside the Olympic training center’s staff trainers, therapists and physicians, I kept one thing in mind…Whatever I do could affect my profession. I knew it was about
more than just myself.

I knew that if If I screwed up or goofed off or played a prank, it could have a negative impact on other sports chiropractors everywhere. If I wasn’t thorough in my consults and exam, if I wasn’t effective in my treatment, if I was disrespectful in any manor it could have a negative impact on my profession.  While at the Olympic Training Center I considered myself an ambassador for my profession. What I did there could reflect positively or negatively on my profession.

As such I have to say to swimmers Ryan Lochte, Gunnar Bentz, Jack Conger and Jimmy Feigen of Team USA (and to all athletes everywhere representing your family, town, county, school, state or country) - you are all ambassadors. We watch you with high hopes and hold you in high regard. We cheer for you when you compete, we ache with you when you lose or get hurt and we feel such pride when you win. You are an extension of us.  You carry the spirit of our country with you wherever you go and what you do on the world stage reflects on the USA and each and everyone one of us here at home.

Concussion In Sports: An Internet TV Show Panel Discussion with Lawrence Taylor & Lisa Dwoskin

Recently I was asked to be part of a panel discussion on the topic of concussion in sports and of course I did it. It was Co-Hosted by exercise physiologist and health coach Lisa Dwoskin & New York Giants Hall of Famer Lawrence Taylor. I was happy to share the spotlight with my esteemed colleague Dr. Steve Perman of West Boca Raton, FL.

Concussion in sports is a deep topic. Think of the mad hatter going down the rabbit hole. You don't know how deep it will go or where it will lead you. Just expect the unexpected. Enjoy the video and post your thoughts and comments below.

The take home message from all this is (1) treat all concussion, even mild ones-seriously. (2) parents with children on a sports team where concussions and other sports injuries are probable, pool some money together and hire an athletic trainer or a certified sports chiropractor to evaluate and manage any injuries. (3) When it comes to concussion, all those bumps on the head add up. Addressing them early by a competent functional neurologist can make all the difference.

Let’s Dispel The Myth: Trauma Can Cause Schmorl's Nodes

It’s not that Schmorl’s nodes are mythical, they actually do occur. The problem is doctors and scientists like to give conditions they discover cute (and sometimes not so cute) names that are really not very descriptive of the physiology of the actual condition. Such is the case with Schmorl’s nodes.

First described by Schmorl in 1927, a Schmorl’s node has classically been known to be due to any process which weakens the cartilaginous endplate or the subchondral cancellous bone. Schmorl’s nodes have been most commonly associated with Scheurman’s disease, Paget’s disease, degenerative joint disease, sickle cell anemia and malignant tumors. Essentially they occur because some physicologic process has gone awry, weakened the bony end-plate of the vertebral body and as the bone weakens, pressure from the disc pushes and deforms upward into the vertebral body.

When physicians see Schmorl’s nodes on x-ray, CT-scan or MRI it is typically brushed off as an incidental finding-regardless if the area is symptomatic or not. Although radiology text books always mention trauma is a possible cause, it’s always at the very bottom of the list of potential diagnoses and usually never given a second thought.  I think this needs to change.
Multi-level Schmorl's Nodes (fracture-herniation)

2 studies published in Spine (V. Fahey et. al.) 1998 and the American Journal of Neuroradiology (Wagner, et. al. 2000) seem to bring the Schmorl’s node phenomenon to the top of the list when there is spinal pain caused by a trauma in an area where a Schmorl’s node is present. These papers show that not only can trauma cause a Schmorl’s node but most (57%) of the Schmorl’s nodes in 2000 AJNR study  were traced to episodes of “significant, sudden-onset, localized, non-radiating back pain and tenderness for which the MRI images showed Schmorl’s nodes surrounded by vertebral body marrow edema”.

Now that we know Schmorl’s nodes are occurring from trauma more often than originally thought, what exactly is a traumatically caused Schmorl’s node?  That’s a very simple answer. With a strong annulus and enough pressure placed on the intervertebral disc from the forces involved in car accidents, falls and other trauma;  the force takes the path of least resistance, fracturing and herniating their way up through the inferior and/or down through superior end plate of the vertebra - fracturing its way into the spongy bone.

Bottom line, if you find yourself having pain after trauma and your x-rays, CT scan, or MRI shows a Schmorl's node(s) in the area of your injury, then you can bet that the Schmorl's node was caused by the trauma. Just rule out the other Schmorl's node-associated conditions first.  Now you just have to find a physician open-minded enough to read the research and work with you to resolve your symptoms.

Dr. Narson's NO GRAIN-NO PAIN Paleo Pancakes

For one reason or another you can't or won't tolerate flour or other grains in your diet but miss some of those fun foods of your past. There are many good reasons to give up grains all of which seem to be forgotten when some of your favorite foods are offered up by friends and family.

If you're on a Paleo Diet or other grain restricted diet and miss your Sunday morning pancakes? Well here's your perfect Paleo replacement.

Dr. Narson's No Grain-No Pain Paleo Pancakes

What you'll need:
  • 1 Large very ripe banana
  • 2 cups of almond flour
  • 1 Tablespoon of baking powder
  • 3 large eggs (I use free range)
  • 1 pinch of sea salt (I actually use Base Salt, the salt I used to prevent cramps during Ironman Miami 70.3 because it has a better mineral balance, not just NaCl)
  • KerryGold butter to coat the pan
  • OPTIONAL: You can add some vanilla extract and a pinch of stevia for additional flavor.
So here we go...

STEP-1
Pour 2 cups of almond flour, 1 Tablespoon of baking powder and 1 pinch of sea salt in a medium size mixing bowl.

STEP-2
Next, add the banana and 3 eggs.

STEP-3
With a stick blender, mix/blend all ingredients until well incorporated into a smooth batter.

STEP-4
Many like to use non-stick pans, but I have a problem with the chemicals in those non-stick surfaces. So, I use a very well seasoned cast iron skillet.  I leave that to your discretion.

Drop in some Kerry-Gold butter to coat the pan, then with a large serving spoon, drop in your batter creating about 4-5" round pancakes.

Cook until browned on the bottom then with a swift spatula motion, get that spatula under the pancake then flip it over to brown the other side. Let cook until fully cooked through.

Serve and enjoy!

Quick Note About Iron Skillets...

If you're using a properly seasoned cast iron skillet, you should simply be able to wipe it clean with a paper towel.

No mess, no fuss, no grains, just good protein rich pancakes that your brain and body will love!

Your Non-Existent Gluten Allergy & Why Your Doctor Is Probably Wrong About It

Another patient came to the office this week and said "my family doctor ran a bunch of blood tests and said I don't have an allergy to gluten, I don't have Celiac Disease so I can eat wheat".

Well, they're probably wrong. Why? Because you can't hit a target you cannot see. In this case, you can't find something you're not even looking (or testing) for.

I've seen it time and time again. What I've come to understand is that most doctors don't understand the gluten problem. If you went to a conventional physician and they ran a gluten sensitivity test to see if you have Celiac Disease, you were test for something called Alpha Gliadin. That's it. This is the classic "Gluten" test doctors have been doing for decades. However since about 1999 scientific research has said there is more, much much more than Celiac disease to test for and there's more you can have an allergic reaction to than simply Alpha Gliadin.

As a matter of fact, research is showing that most gluten reactions happen outside of the gut. Every tissue is susceptible, especially Thyroid, Skin & Central Nervous System.

Here's the moral of the story. If you've had a gluten sensitivity test done recently by your family physician, someone that practice standard medicine, ask them what gluten sensitivity tests were run. If it only tested Alpha-Gliadin, then your test was extremely incomplete. A complete test will test for all of the following components...

Wheat IgG
Wheat IgA
Wheat Germ Agglutinin IgG
Wheat Germ Agglutinin IgA
Native + Deamidated Alpha-Gliadin-33-mer IgG
Native + Deamidated Alpha-Gliadin-33-mer IgA
Alpha-Gliadin-17-mer IgG
Alpha-Gliadin-17-mer IgA
Gamma-Gliadin-15-mer IgG
Gamma-Gliadin-15-mer IgA
Omega-Gliadin-17-mer IgG

Wheat is more than just gluten

If you're not being tested for it,

All of it,

...then you just don't know

Any questions, contact me.

Poor Posture Is Costing Your Company Profits

I'll just come right out with the punch line: Poor posture (yeah, poor posture) is costing your company profits! It's more like a punch to the gut (of your business).  In 2002 Lockheed Martin lost $34 million due to employee illness. Half of that $34 million loss was directly due to employees with neck pain, back pain and headaches.

And guess what? Lockheed Martin isn’t the only one. Statistics show that at any given time, 90 million people are suffering with low back pain and another 109 million with neck pain and headaches. I’ll bet there’s at least a few people at your company distracted by some aches and pains of their own.  If that’s true (and statistically it is) How are you dealing with it so that your company isn’t losing tons of money as well?  If you’re the one working for the company, then how are you able to do the best job you can if you’re in pain? Could you have lost an opportunity for a raise or other advancement?

Although it doesn’t seem like a big deal to most, it comes down to poor performance, poor concentration and poor quality work because pain is distracting you from your work. I’m not simply talking about you lifting something causing an injury or you getting banged up at the gym or playing sports over the weekend. I’m talking about neck pain, back pain and headaches being caused just because you are sitting at your desk doing your normal every day job.  Oh, and guess what? It all started when you were in grade school.

Simply put, if you sit at a desk throughout the day your posture is suffering. The changes sitting causes to your posture results in abnormal stress on your spine, abnormal stress on your supporting muscles and inflammation of little tiny nerve receptors. Add these together and you’re in pain.  And don’t forget about how poor posture looks when you see others. What most people don't even notice is the poor "lower body posture" that causes the lower back pain.

The pain doesn’t have to be severe, a mild dull ache or a little spasm in one of the muscles of your upper back or neck is enough to distract you just enough to lower your level of concentration. Add this together with the other days, weeks, months and years and all that adds up. It certainly did for Lockheed Martin and it does for just about every business out there.

It’s not just sitting at a desk, but any job where you assume a particular posture for prolonged periods of time. The good news is, there are easy strategies to reverse this type of posture involving some simple stretches, exercises and modifications to your work space.  All of which I teach at your company in my 30 minute lunch-n-learn series “Surviving A Day At Your Desk”.

Wouldn’t you rather go home in the evening after work feeling great, full of energy and ready to go out? It’s certainly a lot better than feeling tired and achy with only enough energy to drive home and plop down on the couch.

Want to help yourself and your company? Call me so we can schedule a 30 minute lunch-n-learn at your office.  It’s as simple as clicking this link and asking.

Yours in Health

Is Chiropractic Good For Your Brain?

How important is brain function to you and your life? I’d say it’s pretty important if you do anything more than sit on a couch all day. From the everyday entrepreneur, the high level administrator, CEO of a large corporation, the athlete, the college student and mother of 2 alike, without a fully functioning brain, well, you just wouldn’t be you.

September 18, 1895 D.D. Palmer did a crude adjustment of a “stone deaf” Harvey Lillard and Lillard’s hearing was restored.
DD Palmer adjusting Harvey Lillard, Sept 18, 1895
It’s an old story that “we” chiropractors pass on from one generation of chiropractors to the next.  There have even been cases of blindness being “cured” by chiropractic
1.  Personally, I have experienced relief from migraine headaches as well as episodes of brain fog after receiving a chiropractic adjustment.

How is it that a chiropractic adjustment can affect the brain? Isn’t chiropractic treatment only for back and neck pain? Apparently some new research makes the connection between spinal segmental dysfunction (subluxation) and the frontal cortex of the brain.

This new research study2 shows adjusting dysfunctional spinal segments effects the pre-frontal cortex of your brain AND the results of which last up to at least 30 minutes (the max tested in this study).

Some of the functions of the pre-frontal cortex of your brain are responsible for:
  • Planning complex cognitive behavior
  • Personality expression
  • Decision making
  • Moderating social behavior

Seeing how this research shows that a chiropractic adjustment of a segmental spinal dysfunction can affect the brain, who knows how a chiropractic adjustment can help you.  I mean, if your brain is important to you.

So much for all those that say chiropractic is just about neck pain, back pain and headaches!!!

Chiropractic Care Nearly 6 Times More Effective Than Medical Treatment of Back Pain

Chiropractor Miami Beach - Todd Narson, DC, DACBSP

On June 16, 2009 the Report to Congress on the Evaluation of the Demonstration of coverage of chiropractic Services Under Medicare; was submitted. A survey was conducted of 3,464 people seeking chiropractic care for pain and/or difficulty walking due to back pain (78%), neck pain (50%), Hip Pain (38%) and shoulder pain (32%). Symptoms were described as either "severe" or "very severe" and in 2/3 of the people, their symptoms "interfered considerably" in their usual daily activities. The most frequent type of treatment they received was (1) chiropractic manipulation (aka: adjustments) and (2) various physical therapy modalities like EMS, ultrasound, ice and heat.

Of the 3,464 people surveyed, 71% (2,459) responded to the survey.

Graston Technique®, ASTYM, FAKTR, SASTM, Gua Sha are all forms of IASTM (Instrument Assisted Soft Tissue Mobilization). The purpose of which is to break down scar tissue and adhesions that are causing multi-layered soft tissue restriction and adversely affecting the dynamic function of the underlying muscles, tendons, fascia & fascial planes, ligaments, etc. These restrictive scars and adhesions can be from old injuries, surgeries, repetitive strain (like carpal tunnel), mis-use or improper use of your body when working, playing, doing sports or just from the day-to-day activities of your normal life. They happen from car accident, sitting in front of a computer too much for too long or from childhood bumps and bruises that were never really attended too properly. There are some minor difference between the techniques I mentioned above however ultimately, all are very effective for the right conditions. However effectiveness doesn't depend so much on the original coursework or specific technique, but the experience of the practitioner and the accuracy of their diagnosis coupled with proper application of the technique. One thing I want to "get off my chest" is that Graston Technique® & ASTYM aren't new techniques. Your doctor or therapist may have just learned it and is all excited about it, but these techniques have been around for over 20 years with the original research being done at Ball State University. Gua Sha actually dates back to the 5th &; 7th centuries (roughly 700-BC to 500-AD). FAKTR & SASTM have been around since the mid to late 2000 respectively IASTM techniques are not simply the use of instruments to loosen or remove adhesions and provoke an acute inflammatory response, but it’s the follow up treatment, stretching, therapies, resistance, rehab & anti-inflammatory nutrition that completes the techniques and is equally as important. Otherwise, you're simply doing Gua-Sha/Instrument Massage and without the proper follow-up, you can expect the symptoms and condition to return. Sometimes, they’ll return with a vengeance. This is why you want a fully trained (and experienced) practitioner performing this on you or your athletes Having become certified in Graston Technique® M1 & M2 in 2002 and several years ago, trained in FAKTR-PM (now known simply as FAKTR), 11 years of the last 20 years using IASTM full time in my practice, weekend excursions as a member of the sports medicine team at local sporting events, a 10 day rotation at the 2006 Central American & All Caribbean Games and 10 days at the 2011 Pan American Games, all gives me a unique experience and perspective in the practical application of IASTM. You’ll find many others with varied but similar experience. Some of the most important factors for providing good IASTM are: 1. A good physical exam 2. Sports specific examination of their injury 3. Creativity -Thinking outside the box (and sometimes outside the walls of your clinic) As a result of my experience with Graston Technique®, FAKTR and IASTM in general, I developed my own instrument design known as the Narson Body Mechanic N6. The design is patented in Canada and I’m very proud to announce that it has been awarded a US Patent as well (just received the certificate today!!). The reason I designed this instrument was out of my constant need to be more efficient with IASTM. The design affords me the ability to carry all of the most useful IASTM instruments with me in one convenient tool. No longer do I have to carry a kit of tools with me. No longer do I have to clean multiple instruments between patients. I have everything in one hand at all times. The design includes a non-slip gripping surface which reduces tool slippage and reduces practitioner finger & hand fatigue which, the longer you do IASTM with real highly polished stainless steel instruments, you’ll come to appreciate. The other thing most of us in the USA can appreciate is that it’s the only useful “multi-tool” for IASTM that is made from 100% U.S. steel and made 100% by hand right here in the U.S.A. When it comes to giving effective IASTM, the most important thing to remember is after a good base education, it’s the wealth of experience along with the doctor’s (or therapist) ability to be creative. From my experience, it's thinking outside the box that matters most, coupled with a good nutrition plan, rehab and anti-inflammatory therapies and the results will astound. It’s very possible with IASTM to take long time chronic injuries-treat them with IASTM and have the athlete back out playing the same day or the next day. This year I have helped a multitude triathletes get back into their training to complete in the full Ironman triathlon. Others are back training to complete 26.2 distance marathons and half marathons and some people are back into their workout routine because of the proper application of IASTM. Often you’ll find me at the pool, the track, with a cyclist on an indoor trainer or running on the sidewalk with a patient working with them while they do their sport. For me, putting a person in the exact environment that produces their symptoms is the only way to resolve the problem. Just like anything else in life, experience matters and creative matters. Even with IASTM. 'nuff said Dr. T

Plantar Fasciitis in Runners & Triathletes

Plantar Fasciitis in Runners & Triathletes

Typically seen in runners and triathletes plantar fasciitis is characterized by pain at the bottom of the foot when standing, walking, jogging or running. Often the person notices pain on the bottom of the foot when they get out of bed first thing in the morning as they put their feet on the floor and stand up.
Although plantar fasciitis can be a result of a one time injury, faulty biomechanics is typically the culprit. Excessive external hip rotation during foot strike causing over pronation causes increased elongation to the point of micro or macro tearing anywhere along the plantar fascia to its calcaneal (heel) attachment.
When the plantar arch is lowered due to pronation it causes increased traction forces to the fascia. With repetitive foot strikes, tissues along the plantar fascia become over stressed, inflamed and can eventually fail resulting in plantar fasciitis. In some cases the plantar fascia is strong enough to withstand the repetitive forces and instead of plantar fasciitis, excessive traction forces on the calcaneal attachment of the plantar fascia results in a traction spur known commonly known as a “heel spur”. A heel spur can easily be identified on ultrasound or x-ray examination. It is safe to say that eventually, most cases of plantar fasciitis will lead to heel spurs if the condition is not resolved and faulty biomechanics are not corrected.
In our office we have been able to successfully manage most cases of plantar fasciitis using conservative treatment methods such as IASTM (Instrument Assisted Soft Tissue Mobilization), pulsed ultrasound, interferential stimulation, ice & heat compresses, kinesiology tape and manipulation to the joints of the foot and ankle. On rare occasions, the athlete needs to be referred to another specialist.

Rehabilitation for plantar fasciitis should also include strengthening and increasing the resiliency of the involved tissues.  Correcting faulty running biomechanics with a running or triathlon coach is a must and along with your sports doctor (hopefully me), most cases of plantar fasciitis can be successfully addressed and plantar fasciitis a thing of the past.
